PPE stands for Personal protective equipment. It is the protective equipment usually consists of standard precautions - which includes gloves, mask, and gown, however, if there are blood and airborne infections then, PPE kit will include: Face protection, goggles, mask/respirators, face shield, gloves, gown or coverall, headcover, and safety shoes.

Components of PPE Components of PPE

  1. Face shield

Contamination of mucous membranes of the eyes, nose, and mouth is likely in a scenario of droplets generated by cough, sneeze of an infected person. So the protection of the mucous membranes of the eyes/nose/mouth by using face shields/ goggles is an integral part of standard and contact precautions.

  1. Masks/ Respirators

Mask / Respirators help to protect mainly the upper and lower respiratory tracts. So protecting the airway from the particulate matter generated by droplets/aerosols prevents human infection. Contamination of mucous membranes of the mouth and nose by infective droplets or through a contaminated hand also allows the virus to enter the host. Masks/Respirators are worn as droplets and airborne precautions while dealing with a suspect or confirmed case of COVID-19 or while performing aerosol-generating procedures. Masks are of different types. The different type of mask to be used is related to the particular risk profile of the category of personnel and his/her work. There are two most important types of masks which are recommended for various categories of personnel working in hospital or community settings, depending upon the work environment:

  1. 3 Ply medical mask

3 Ply medical mask is a disposable mask, fluid-resistant, provide protection to the wearer from droplets of infectious material emitted during coughing/sneezing/talking. It can be used by general public.

  1. N-95 Respirator mask

N-95 Respirator mask is a device with high filtration efficiency to airborne particles. To provide the complete air seal to the wearer, which has 0% inward leak such masks are designed to achieve a very close facial fit. N-95 mask should have high fluid resistance, good breathability (preferably with an exhalation valve), clearly identifiable internal and external faces, an aluminum wire on upper side to give a better fit at the bridge of the nose, duckbill/cup-shaped structured design that does not collapse against the mouth. The filtration capacity of N-95 masks exceeds those of triple layer medical masks. Since N-95 provide a much tighter air seal than triple layer medical masks, they are designed to protect the wearer from inhaling airborne particles and are thus recommended to be worn by frontline workers or healthcare professionals.

  1.
  2. Gloves

Gloves are used when a person touches an object/surface contaminated or suspected to be contaminated by COVID-19 infected person. Therefore, to avoid direct contact with the coronavirus and hands, gloves are worn. As there are chances that a person can accidentally touch any infected area and then touch his own eyes, nose, or mouth and he may get exposed to the virus, it is not thought to be a predominant mode of transmission, however, necessary care should be exercised while handling objects/surface potentially contaminated by suspect/confirmed cases of COVID-19.

  1. Coverall/gowns

Overall cover or gowns are used to protect torso of healthcare providers from exposure to virus. Although the gowns typically provide 360-degree protection as they are designed to cover the whole body, including back, lower legs, head and feet as well, the design of the medical/isolation gowns do not provide continuous whole-body protection as there are possible openings in the back or just coverage to the mid-calf is only provided. Using the appropriate protective equipment, it is possible to create barrier to eliminate or reduce contact to droplet or airborne exposure, both known to transmit COVID-19, thus protecting healthcare professionals working in close proximity of suspect/confirmed COVID-19 cases or their secretions. Coveralls and gowns are deemed equally acceptable as there is a lack of comparative evidence to show whether one is more effective than the other in reducing transmission to health workers. Gowns are considerably easier to put on and for removal.

  1. Shoe covers

Shoe covers are made up of impermeable fabric to be used overshoes to facilitate personal protection and decontamination.

  1. Headcovers

Coveralls usually cover the head. The gowns being used should use a headcover that covers the head and neck while providing clinical care for patients. Hair and hair extensions should fit inside the headcover

When a PPE should be used

All staff, patients, and visitors are recommended to use PPE when there will be contact with blood, bodily fluids or respiratory secretions. For healthcare workers providing direct care to coronavirus patients, WHO guidelines mention the use of Medical mask, Gown, Gloves, and face protection (goggles or face shield). If these healthcare workers are performing Aerosol-generating procedures on COVID-19 patients, WHO recommends the use of Respirator N95 or FFP2 standard mask, or equivalent, Gown, Gloves, Eye Protection, and Apron.

Why PPE is important

PPE is important in a number of ways:

1- It helps in minimizing the spread of the highly contagious coronavirus disease from one person to another.
2- It protects the lungs of a healthy person while breathing in contaminated air.
3- PPE also provides protection to the eyes of the healthy person from the microorganisms in the contaminated air.
4- PPE provides protection to the skin from coming in contact with the infected materials.
5- Coveralls are made up of impermeable fabric which acts as a barrier between infectious materials and the body parts along with the skin.

It also provides protection to healthcare professionals who are on the frontline treating COVID-19 hit patients. COVID-19 Coronavirus disease is highly contagious and spreads mainly through respiratory droplets, either by direct touch or through contaminated objects.
